Lifestyle Change Program – Prevent Type 2 Diabetes

Are you 45 Years of Age or Older?
Are you Overweight?
Do you have a Family history of type 2 diabetes?
Are you physically active fewer than 3 times per week?
Did you have gestational diabetes?
If you answered yes to 1 or more of these questions you are at risk for developing Type 2 Diabetes.

The Prevent T2 Program lifestyle change program can help you!
This no cost program will be offered in Callahan, Florida at the MCCI Building. Beginning August 30 we will meet from 11am to 12pm. We will meet every Wednesday until November 15. Lunch will be provided, at no charge, after each class.
